If I select to run UltraVNC as a SERVICE, what will that do?

It will run UltraVNC server as a Windows service.
That means it will always be running, and will get restarted (more or less silently) whenever the machine is booted and regardless of who logs in, or even when no one is logged in. You no longer have to intentionally launch the VNC server.
You could think of it as a "background task".
This also means that your machine will always be reachable via VNC viewers.
